A new Registration Form (HSCR1) was introduced on 1st June 2017 and must be completed by any new patient from outside Northern Ireland wishing to register with a GP Surgery. This includes patients moving from England, Scotland or Wales to reside in Northern Ireland OR patients who have come from other EEA countries or overseas. This also includes someone who was born in Northern Ireland but moved away and has now returned to take up residence again.

Please be aware we unable to accept an application to register with the practice if supporting documentation is not supplied with the application from LIST 1, LIST 2 and LIST 3.
